Lack of consideration for human factors led to in-flight breakup of SpaceShipTwo

The The National Transportation Safety Research Board (NTSB) recently determined that a failure to consider human factors including pilot error was the cause for the in-flight breakup of SpaceShipTwo, this past October. The NTSB is charged with determining the probable cause of transportation accidents among other safety related goals and plays a crucial role in advocating for safety in the form of human factors assessments and human-systems integration.
