How Does Human Behavior Lead to Surgical Errors? Researchers Count the Ways

Researchers at the Mayo Clinic identified factors contributing to "never events" or events that shouldn't ever happen, like wrong site surgeries or putting in a wrong implant. They estimated that the rate of never events was around 1 in 22,000 procedures, with 628 environmental, organization or individual characteristics that led to overall errors.
